From the description, it sounds suspiciously like the same one that has been making the rounds on the internet.
The first pictures only showed the break in the frame, and yeah, I would agree that the weld is not the greatest. Later pictures showed more of the damage on the bike. Forks separated from both triple clamps (probably both broken - no other way for them to separate like that), front wheel ripped apart (hub no longer together with the outer rim), etc.
I tend to suspect that there was a catastrophic impact with a fixed object (wall?), and that the condition of that weld was basically irrelevant because if it didn't break there, it would have broken somewhere else. You cannot design or manufacture something to reasonably survive an impact like that ...
