Wouldn't going -1 or +2 on the gearing give you the same effect on midrange versus a full exhaust at the fraction of the cost?

Not exactly. Changing the gearing of the bike cause it to run at a higher engine speed given the same road speed. It brings you higher into the powerband. Yes, you will have more power, but its because you are now at a higher RPM, not because the midrange is improved. Thats not to say you dont feel the effect akin to imrpoved midrange though.
